Checklist for Agent Petitioners

  • Contract between Agent and Beneficiary stating the terms of the agent-beneficiary relationship. See sample here.
  • Petitioner support letter (our office will work with you to draft this).
  • Itinerary that lists jobs or events for the duration of the visa. See sample here.
  • Information about the agency, like a brochure or annual report
  • Information for forms:
    • Main address of business
    • Federal Tax ID #
    • Year founded
    • Gross and net annual income
    • Total number of employees
    • Full name of your company
    • Name and title of designated official (the person who will sign forms, etc.)
    • Phone, fax and email for designated official
    • Name, phone, fax and email of contact person (if different from designated official)
  • If the Beneficiary has itinerary events with other employers, we will also need the following for each event:
    • Agent-Employer contract: this must be signed by both the employer and the Agent Petitioner. See sample here.
    • Contract between the employer and the Beneficiary. This can be any document confirming the terms of employment including emails, a signed oral agreement, or a formal contract
    • Information about the event and any paperwork evidence, such as email invitations, online listings, etc.
    • Basic information about the employer

Checklist for Employer (Non-Agent) Petitioners

  • Contract or Oral Agreement between Employer and Beneficiary stating the terms of employment (please contact us if you need a sample)
  • Petitioner support letter
  • Complete job title and detailed job description
  • Information about the employer, like a brochure or annual report
  • Information for forms:
    • Main address of business/place of work and address where employee will work
    • List all physical locations where the prospective employee will work
    • Salary offered to the beneficiary
    • Minimum number of hours per week that the Beneficiary will work
    • Is the position full time or part time?
    • Dates of intended employment (must not exceed 3 years)
    • Federal Tax ID #
    • Year founded
    • Gross and net annual income
    • Total number of employees
    • Full name of your company
    • Name and title of designated official (the person who will sign forms, etc.)
    • Phone, fax and email for designated official
    • Name, phone, fax and email for contact person (if different from designated official)
